HPI: Patient presents to the office today with complaints of a rash to both upper arms for 4 days.
Was working outside in the garden.
No known contact with poison but was pulling weeds.
Complaints of itching. Has tried over-the-counter hydrocortisone. Helps for a short period.

PMH: HTN-hyperlipidemia

Family history: HTN in father

Social history: nonsmoker, nondrinker

Allergies: NKA

Medications: Lisinopril 10mg daily
Simvastatin 20mg daily

ROS
General: no fevers, chills
Skin: red rash to bilateral arms, + itching
MSK: no myalgias

Objective
Physical exam
General: WD/WN BP 110/60, pulse 72, respirations 18, temp 98

Skin: bilateral forearms with red/brown flat circular areas some with central clearing, no
increased warmth or edema to arms
