Imagine that you have been appointed the Director of Health at the Kaluyu Memorial Hospital in Nairobi, Kenya – a for-profit hospital. The facility is also a referral hospital and receives severe cases of accidents, chronicle / communicable diseases, and houses an HIV / AIDS ward. As you settle into your position, you realize that the employees always act scared as they approach their superiors. Some of the employees even deliver files and leave your office in a hurry.

As you make your routine departmental visits, you realize that there are tensions between nurses and doctors, and there is a sentiment that the nurses tend to do the majority of the work within patient care but the doctors get all the credit. You realize that the employees are always looking forward to the end of their shifts to go home. Some employees even come back to work wearing the same unwashed clothes as the previous day. Too many employees are calling in sick, and many of them give weak reasons for their tardiness.

There is also a sense that employees and nurses dominate other employees in similar positions. In meetings and conference calls, some employees are always quiet and never participate. You notice that people with families tend to gather and talk quietly on breaks. The new mothers working for the hospital have to use bathrooms to pump breast milk for their infants and the refrigerators do not work well. Looking at the financial statements of the hospital, you realize that the hospital expenses are higher than the industry standard, and it makes losses year after year.

Write a four to six (4-6) page paper in which you:

  1. Investigate the major      demotivational factors at Kaluyu Memorial Hospital. Assess the level of impact      that the identified factors could have on specific areas of service and      business if the administration does not address the situation. The EBP Project provides a significant opportunity for furthering the nursing profession. Formulating a specific, applicable question is a necessary first step for developing your project. The question defines the focus of the EBP Project and guides your selection of research methods and tools. Think about aspects of your specialty area and begin to formulate a question that relates to your nursing practice.

The primary purpose of this Discussion is to provide a forum in which you and your colleagues can support each other in cultivating a practice-related question.

To prepare:

  • Develop      your EBP Project question using the PICO (patient group, issue      or intervention, comparison, outcome) method as described in Chapter 2      of Clinical Research for the Doctor of Nursing Practice (p.      29) and other resources. Depending on your specialty area, you may modify      the PICO method as necessary to be appropriate for your target population.
  • Begin      to complete the “Literature Review Matrix,” reviewing at least four to six      research articles relevant to your specialty area and      your EBP Project question. As revenue generators, NPs must be aware of how their work contributes to the overall revenue of the clinical practice. You see 20 patients per day on average and take call every third weekend. According to Buppert (2011), an NP who sees 15 patients per day at $56 per patient visit, on average, brings in $840 per day. Allowing 1 week off for continuing education, 1 week off for illness, and 4 weeks off for vacation, this NP will bring in $193,200 a year, potentially. However, not all bills are paid. With a 90% collection rate—a reasonable collection rate for an efficient practice—this NP actually will bring in $173,880 per year. An NP who sees 24 patients per day will bring in $1344 per day, or $309,120 per year in accounts receivable. With a 90% collection rate, this NP will bring $278,208 to the practice (Buppert, 2011).

Establishing a salary can be a challenge for NPs. Deducting 40% of the NP’s gross generated income for overhead expenses (rent, benefits, continuing education, supplies, malpractice, lab expenses, and depreciation of equipment) leaves $104,280 for the 15-patient-per-day NP and $166,925 for the 24-patient-per-day NP. Further deducting 15% of that figure to pay a physician for consultation services leaves $88,638 in salary for the 15-patient-per-day NP and $141,887 in salary for the 24-patient-per-day NP. Deducting 10% for employer profit leaves $79,775 in salary for the 15-patient-per-day NP and $127,699 for the 24-patient-per-day NP (Buppert, 2011).

What salary would you propose for the contract renewal? How does your salary proposal fit in with the community standard for an NP in a similar practice? Use logical reasoning, and provide evidence based rationales for your decisions. Keep in mind that your negotiation terms and conditions must be within the legal scope of practice for an ANP.

The purpose of this paper is to complete a comparative ethical analysis of George’s situation and decision from the perspective of two worldviews or religions: Christianity and a second religion of your choosing. For the second faith, choose a faith that is unfamiliar to you. Examples of faiths to choose from include Sikh, Baha’i, Buddhism, Shintoism, etc.

In your comparative analysis, address all of the worldview questions in detail for Christianity and your selected faith. Refer to Chapter 2 of Called to Care for the list of questions. Once you have outlined the worldview of each religion, begin your ethical analysis from each perspective.

In a minimum of 1,500-2,000 words, provide an ethical analysis based upon the different belief systems, reinforcing major themes with insights gained from your research, and answering the following questions based on the research:

  1. How would each religion interpret the nature of George’s malady and suffering? Is there a “why” to his disease and suffering? (i.e., is there a reason for why George is ill, beyond the reality of physical malady?)
  2. In George’s analysis of his own life, how would each religion think about the value of his life as a person, and value of his life with ALS?
  3. What sorts of values and considerations would each religion focus on in deliberating about whether or not George should opt for euthanasia?

  5. Finally, present and defend your own view.
